Due to the background check related violations cited in today?s inspection, the provider is required to follow-up with the appropriate department regarding the status of any missing and/or incomplete background checks. Documentation of the completed follow-up must be sent to the Licensing Inspector within 10 business days, and a copy of all missing/incomplete background check results must be sent to the Licensing Inspector upon arrival.

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A virtual, monitoring inspection was initiated on 11/30/2020 and concluded on 12/1/2020. The in-charge person was contacted by telephone to initiate the inspection. There were 25 children present and 4 staff. The inspector emailed the in-charge person a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 63.2(17)-1720.1-B-3 Description: Based on a review of staff records, a staff record did not contain documentation of out-of-state central registry requests.
Evidence: Staff B's (date of hire: 7/13/2020) record indicated that the employee has resided in Arizona and Kansas within the past five years. The employee's record did not contain documentation of a central registry request for either state.Plan of Correction: We will reach out to the states involved requesting out-of-state background check results.
Standard #: 63.2(17)-1720.1-B-4 Description: Based on a review of staff records, 2 staff records did not contain documentation of out-of-state criminal history requests.
Evidence:
1. Staff B's (date of hire: 7/13/2020) record indicated that the employee has resided in Arizona and Kansas within the past five years. The employee's record did not contain documentation of a criminal history search for Arizona.
2. Staff C's (date of hire: 8/1/2020) record indicated that the employee has resided in California within the past five years. The employee's record did not contain documentation of a criminal history search for California.Plan of Correction: We will reach out to the states involved requesting out-of-state background check results.
