Standard #: 22VAC40-665-170-A Description: Based on the review of children's records, it was determined that the vendor does not have a written or electronic record for one child in care. Evidence: The vendor has one child that started trial care ( two hours a day) yesterday. The vendor failed to have any records for the child. Plan of Correction: I will give her paperwork to fill out when she comes to pick her daughter up.
