Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on December 3, 2020 and concluded on December 3, 2020. The director was contacted by telephone to initiate the inspection. There were 82 children present and 12 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-665-520-B-11
Description: Based on review of children's records and interview, the facility did not obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence:

1. Child #6 (enrolled 04/30/2018) with a diagnosed food allergies, did not have a written allergy care plan on record.
2. Child #7 (enrolled 01/27/2020) with a diagnosed food allergy, did not have a written allergy care plan on record.
3. The administrator acknowledged child #6 and child #7 did not have allergy care plans to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: Per the director: I will have each parent obtain an allergy care plan from their physician.
