A joint investigation was conducted with the director and local agency regarding an allegation of lack of supervision and failure to notifying parent of a minor injury. The investigation was initiated on October 1, 2020 and concluded on October 6, 2020. This investigation was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. Interviews were conducted and facility documentation was reviewed. The allegation regarding supervision was determined to be not valid. The allegation regarding notifying parent of minor injury was determined to be valid.

Standard #: 22VAC40-185-550-L Description: Based on staff interview and review of documentation, the center failed to notify the parent of any known minor injuries by the end of the day. Evidence: On September 30, 2020 Child A was injured while at the center and the parents did not receive notification by the end of the day. Plan of Correction: All staff were retrained on proper notification and documentation for when injuries occur within care. Parents received notification on the next business day.
