Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A subsidy health and safety inspection was initiated on 12/7/2020 and concluded on 12/10/2020. The director was contacted by email to initiate the inspection. There were 21 children present and 5 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance with subsidy regulations, and a violation was documented on the violation notice issued to the facility.

Description:	Based upon review of three staff records, the facility has not ensured that they have obtained documentation that staff have been evaluated by a health professional and are determined to be free of communicable tuberculosis and that the documentation was submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department. Evidence: 1. The record provided for staff 2 indicated a date of employ of 11/13/2020. Documentation indicated that the tuberculosis screening was obtained on 8/22/2020, which is not within the last 30 calendar days prior to the date of employment. 2. The record provided for staff 3 indicated a date of employ of 11/15/2020. Documentation indicated that the tuberculosis screening was obtained on 8/12/2020, which is not within the lasy 30 calendar days prior to the date of employment.
Plan of Correction:	The facility responded with the following: In the future, all new employees will be required to obtain tuberculosis within thirty days prior to employ or working with the children.
