The signed Sworn Disclosure Statement or Affirmation form is to be completed by an applicant PRIOR TO hire so an employer can ascertain any potential barrier crimes or any need for out of state background checks prior to making hiring decisions. This was reviewed with the program director.
The need to document follow up regarding Central Registry Checks with the Office of Background Investigations (OBI) within the employee's first 30 days or the employee must be denied employ was reviewed with the program director.

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring/renewal inspection was initiated on 12/7/2020 and concluded on 12/10/2020. The director was contacted by email to initiate the inspection. There were 21 children present and 5 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 3 staff records submitted by the facility to ensure documentation was complete.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
