63.2(17) License & Registration Procedures

Comments:
A renewal inspection was initiated on 12/9/20 and concluded on 12/15/20. The director was contacted by telephone to initiate the inspection. There were 17 children with 2 staff present.

The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children,3 staff and 3 board officer's files.

The information gathered during the inspection determined there were two violations with applicable standards or law.

Violations:
Standard #: 22VAC40-185-260-A
Description: Based on interview, it was determined that there has not been an annual fire inspection report.
Evidence:
The last documented fire inspection report is dated 5/17/18.

Plan of Correction: We will find a more up to date report or contact our local fire official to complete an inspection.

Standard #: 63.2(17)-1720.1-A
Description: Based on a review of three staff and 3 board member's files, it was determined that one staff had not obtained a repeat background check every five years.
Evidence:
Staff A's Central Registry Finding Report is dated 7/1/15.

Plan of Correction: We will obtain a new background check.
