This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 12/4/20 and concluded on 12/9/20. The director was contacted by telephone to initiate the inspection. There were 41 children and 12 staff present.
The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children and 2 staff records to ensure documentation was complete.
The information gathered during the inspection determined there were 3 violations with applicable standards or law.

Standard #: 22VAC40-185-130-A Description: Based on a review of two children's files, it was determined that one child did not have an immunization record on file prior to attendance.
Evidence:
Child A enrolled in 8/20 and does not have an immunization record on file.Plan of Correction: We will get a copy of the immunization record.
Standard #: 22VAC40-185-140-A Description: Based on a review of two children's files, it was determined that one child did not have a physical on file within 30 says of enrollment.

Standard #: 22VAC40-185-260-B Description: Based on a review of the most recent health inspection report, it was determined that a health inspection has not been obtained on an annual basis.
