Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on December 8, 2020 and concluded on December 8, 2020. The director was contacted by telephone to initiate the inspection. There were 42 children present and 5 staff. The Inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a review of records and interview on December 8, 2020, the center did not ensure to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence: 1. The record of Child #1 (DOE 11/09/20) contained documentation of immunizations dated 11/17/20.
2. The record of Child #2 (DOE 10/26/20) contained documentation of immunizations dated 11/03/20.
3. Administration said she thought the rule was 30 days.

Plan of Correction: Be sure to have immunizations before the child starts. May only wait 30 days on physical not shot records.

Standard #: 22VAC40-185-70-A
Description: Based on a review of records on December 8, 2020, the center did not ensure that each staff record contains all the required information.
Evidence: The record of Staff #1 contained telephone reference checks that were not signed by the person making the call.

Plan of Correction: Corrected on site

Standard #: 63.2(17)-1720.1-A
Description: Based on a review of records and interview on December 8, 2020, the center failed to obtain central registry search results every five years for all employees.
Evidence: 1. The records of Staff #2 (DOH 01/29/06) and Staff #3 (DOH 09/09/11) contained documentation of central registry results dated 11/03/15.
2. Administration stated the wrong background check was redone.

Plan of Correction: All staff have now been sent for the CPS.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on a review of record and interview on December 8, 2020, the center did not ensure to obtain a fingerprint based national criminal record check prior to the first day of employment for each staff.
Evidence: 1. The record of Staff #1 (DOH 10/26/20) did not contain documentation of a fingerprint based national criminal record check.
2. Administration stated she went to Fieldprint and was given the document provided for the inspection, which was the Virginia Criminal Record Check. Administration also provided another background check from the Federal Bureau of Investigation that states it is not intended for licensing.

Plan of Correction: Appointment for correct fingerprint have been made.
