Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 11/14/20 and concluded on 11/15/20. The director was contacted by telephone to initiate the inspection. There were no children present as the facility is closed. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a review of two children's records, it was determined that the facility did not ensure that the center obtains documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence:
1. The record for child #2 did not contain documentation of an immunization record.
2. Staff #4 reviewed the record for child #2, and confirmed there was no immunization record available for viewing during the inspection.

Plan of Correction: The facility responded: We will contact the parents of child #2 to obtain an immunization before she returns to care. All enrolled children will have a current immunization record in their file.

Standard #: 22VAC40-185-140-A
Description: Based on a review of two children's records, it was determined that the facility did not ensure that each child in attendance had a completed physical within one month of attendance.

Evidence:
1. The record for child #2 did not contain a physical examination.
2. Staff #4 confirmed that the record for child #2 did not contain a physical examination, and child #2 had been enrolled for more than 30 days.

Plan of Correction: The facility responded: We will contact the parents of child #2 to obtain a physical examination before she returns to care. All enrolled children will have a physical examination in their file.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on a review of three staff records, it was determined that the facility did not deny continued employment of a staff who did not have a search of the central registry finding within 45 days of employment from any other state in which the employee has resided in the preceding five years.

Evidence:
1. The record for staff #3 did not contain documentation of a completed search of the central registry finding from Alabama. The sworn statement indicated staff #3 had previously lived in Alabama within the last 5 years.
2. Staff #4 (Program Director) review the record for the staff #3 and confirmed that the search of the central registry finding from the state (Alabama) in which staff #5 previously resided in has not been received.

Plan of Correction: The facility responded: The background check for Alabama had been previously sent out. We contacted the office responsible for completing the background check, and they said they never received it, so we completed a new check and will forward the results to the Licensing Inspector once received. All new employees who have lived outside of Virginia within the last five years will complete a search of the central registry for that state within 45 days of employment.
