The local agency representatives and licensing inspector conducted an unannounced self-report inspection on 02/11/2020 between 2:00 pm to 3:00 pm in response to a complaint that was received by licensing on 02/03/2020 related to allegations in the area of programs and staffing and supervision of the standards. During the inspection, the licensing staff conducted interviews with staff, completed observations at the center, reviewed staff and children records, and other pertinent documentation and evidence. There were violations cited as a result of this self report incident. See the violation notice for more details. Standard #: 22VAC40-185-410-1 Description: Based on interview with staff, documentation, and photographs, the center did not ensure all staff refrain from all forbidden actions.
Evidence:
1.Interview with staff confirmed the incident at the center on 2/11/2020 involving Child#1 (age 5) sustaining a visible red mark on his upper left arm after physical contact was used by Staff #1.
2. Staff#1 revealed Child#1 would not take a nap or stay on their cot during nap time. Staff#1 disclosed grabbing Child#1 by the upper left arm to assist back onto their cot. Staff #1 reported the Child#1 dropping to the floor while holding the arm still. Staff#1 stated Child#1 was visibly upset and reported their arm was hurting. Staff#1 acknowledge a red mark on Child#1's arm and then immediately sought first aid and notified the director of the incident.
3. A review of photographs of Child#1's arm injury on the date of the incident, 2/11/2020.
4. A review of documentation on the center's termination notice outlined a violation of the policy for forbidden actions by Staff#1.Plan of Correction: Not available online. Contact Inspector for more information.
