Reviewed background requirements for all agents and employees to include new background requirements, effective 7/1/2020, for individuals who reside outside of the state of Virginia, or who have resided outside of the state of Virginia in the past 5 years.
- Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 12/18/2020 and concluded on 12/18/2020. The director was contacted by telephone to initiate the inspection. There were 35 children present and 6 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-185-60-A Description: Based on record review, 1 of 3 children's records did not contain all required information.
Evidence:
1. The record for Child #1 did not contain the address of two designated people to call in an emergency if a parent cannot be reached.Plan of Correction: I will have the parent update the information during pick-up.
Standard #: 22VAC40-191-60-B Description: Based on record review, a sworn statement or affirmation was not completed prior to the first day of employment for 1 staff.
Evidence:
1. Staff #1 began employment on 9/21/2020.
2. The sworn statement in the record for Staff #1 was dated 12/02/2020.Plan of Correction: Staff #1 completed a sworn statement on 12/02/2020. In addition, a new sworn statement was completed on the most up to date form located on the VDSS website during the inspection.
