Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was conducted on 12/30/2020. The director person was contacted by telephone to initiate the inspection. There were 14 children present and 3 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-510-A
Description: Based on staff interview, it was determined that an emergency prescription medication was on site for a child in care without a signed medication authorization form.

Evidence: Child #1 has food allergies and has an emergency prescription medication on site. There is not a signed medication authorization form completed and signed by the parent.

Plan of Correction: Parent will receive/complete medication and allergy care plan form. Due by Jan. 15th 2021. Parent will sign short term form.
