Evidence: Child #1 is allergic to dairy and nuts and does not have an allergy care plan on file with instructions from a physician with steps to be taken in the event of a suspected or confirmed allergic reaction.Plan of Correction: Parent will receive form 12/30/20. Form is due 1/15/21.
