Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on December 18, 2020 and concluded on December 21, 2020. The in-charge person was contacted by telephone to initiate the inspection. There were no children present and one staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two children?s records and two staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:

Standard #:	22VAC40-185-260-A
Description:	Based on review of documentation and interview, the facility did not obtain an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: 1.The most recent fire inspection report was dated 09/27/2018, exceeding the annual requirement. 2. The administrator stated they did not have an updated fire inspection report.
Plan of Correction:	Per the administrator: We will contact the school and request documentation of a current fire inspection report.
Standard #:	22VAC40-191-60-C-2
Description:	Based on review of staff records and interview, the facility did not obtain a central registry check within 30 days from employment for one staff member. Evidence: 1. Staff #2, with a re-hire date of 06/16/2020, had a central registry check dated 08/04/2018 exceeding 90 days prior to re-hire. 2. The administrator stated a new central registry check had not been requested at the time of rehire for staff #2.
Plan of Correction:	Per the administration: We will have a new central registry completed for the staff.

Description:	Based on review of staff records and interview, the facility did not obtain satisfactory results of the fingerprint based national criminal background check prior to hire for one staff. Evidence: 1. Staff #2, with a re-hire date of 06/16/2020, had a national fingerprint check dated 05/29/2018 exceeding 90 days prior to re-hire. 2. The administrator stated a new national fingerprint check had not been requested prior to rehire for staff #2.
Plan of Correction:	Per the administrator: We will obtain an updated fingerprint background check for the staff.
