Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on January 12, 2021 and concluded on January 12, 2021. The director or in-charge person was contacted by telephone to initiate the inspection. There were thirty-one children present and seven staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed three children?s records and seven staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description:	Based upon review of records, one children's record does not include all required information. Evidence: 1. One of three records reviewed was missing the documentation of having viewed the proof of birth document within 7 days of first attendance. 2. Child #3 first date of enrollment was 03/01/2019.
Plan of Correction:	I will contact the parent to review the document today. The program administrator will ensure that all new children that enroll will have proof of identification within 7 days of attendance.
