This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A complaint inspection was initiated on January 20, 2021 and concluded on January 20, 2021. A complaint was received by the department regarding allegations in the areas of building maintenance and supervision. The director was contacted by telephone to conduct the virtual investigation.
The evidence gathered during the investigation supported the allegations of non-compliance with standards or law and violations were issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Standard #: 22VAC40-185-270-A Complaint related: No Description: Based on observation, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence:
1. In the school age classroom there was an extension cord stretched and dangling from the wall.
2. In the school age classroom there was a loose extension cord running under the children's table, posing a tripping hazard to the children.Plan of Correction: Corrected while Licensing Inspector was on site.
