Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A non mandated monitoring inspection was initiated on 1/04/2021 and concluded on 01/21/2021. The provider was contacted by telephone to initiate the inspection. There were 37 children present. The inspector emailed the director a list of items required to complete the inspection. Four children's records and four staff's records were reviewed.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of four children's records, the center failed to ensure that all children enrolled had a physical with in 30 days of enrollment.

Evidence:
1.Child # 1 and Child # 2 did not have a physical examination. The start date for child # 1 was 5/14/2018 and child # 2 start date was 7/17/2020.

Plan of Correction: The director stated once the program became a licensed facility the current parents were given a timeframe to obtain the missing physicals. The parents will be reminded and given a new date to obtain the physical exam.

Standard #: 22VAC40-185-60-A
Description: Based on review of four children's records, the center failed to ensure that all records were complete.

Evidence:
1. Child # 1 was missing the employment information for the parents or guardians.
2. Child # 2 was missing the following: chronic physical problems, parent or guardians employment information, one of the emergency contact was missing the phone number and the physician's information.
3. Child # 4 was missing the following: previous child care, guardians employment information, and unauthorized persons to pick up.

Plan of Correction: The director has hired a new staff person to help with completing records and reviewing all on a regular basis.

Standard #: 22VAC40-185-90--A
Description: Based on review of four children's records, the center failed to ensure that all three parental agreements were signed by the parent.

Evidence:
1. All four children's records reviewed did not have two of the three required parental agreements.

Plan of Correction: The newly hired staff person will add the two missing agreements to the enrollment form and will have all previous parents and new parents.
