Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on January 29, 2021 and concluded on January 29, 2021. The provider was contacted by telephone to initiate the inspection. There were zero children present and two staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed two children?s records and two staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on review of children's records, the provider did not ensure each record contained documentation of immunizations prior to attending the child day center.

Evidence:
The record for child #2 (date of enrollment 9/10/19) did not include documentation of immunizations.

Plan of Correction: Upon reopening, all members will be asked to provide updated physicals and immunization records. To prevent this noncompliance, members will not be able to begin the program until the paperwork has been returned to the facility. The facility representative, currently Jade Tabb will be responsible to ensure this takes place.
