Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 2/11/21 and concluded on 2/16/21. The director was contacted by telephone to initiate the inspection. There were 55 children present with 12 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 4 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-540-C
Description: Based on observation and interview, the center failed to ensure on each floor of the building used by children a first aid kit contains all the required components.

Evidence:

1. The first aid kit on first and second floor was observed. Neither kit contained triangular bandages.
2. The director stated she was unaware the first aid kits did not contain triangular bandages.

Plan of Correction: At least two triangular bandages will be obtained for each of the first aid kits.

Standard #: 22VAC40-185-540-E
Description: Based on observation and interview, the center failed to ensure the required emergency supplies were operable.

Evidence:

1. The battery operating radio did not have batteries.
2. The director was not aware the radio did not have batteries.

Plan of Correction: Batteries will be purchased for the radio.

Standard #: 22VAC40-185-550-D
Description: Based on record review and interview, the center failed to implement a monthly practice evacuation drill.

Evidence:

1. A review of the documentation of the monthly practice evacuation drill was conducted. There was no documentation of a drill being conducted for the months of June and August in 2020.
2. The director stated staff were unsure how to conduct the drills safely due to COVID and the director was out those months.

Plan of Correction: Monthly practice evacuation drills have been resumed and will be continued each month.

Standard #: 63.2(17)-1720.1-A
Description: Based on record review and interview, the center failed to obtain repeat background checks every five years.

Evidence:

1. The record for staff 4 contains documentation of a central registry background check dated 11/16/15.
2. The director verified the 11/16/15 central registry is the most current one the center has for staff 4.

Plan of Correction: A central registry background check will be completed for staff 4. A review of staff records will be completed to ensure all staff background checks are current. In the future staff background checks will be updated every five years as required.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review and interview, the center failed to obtain fingerprint-based criminal history check determination letters prior to the first day of employment.

Evidence:

1. The record for staff 1 documents the hire date as 8/7/20. The fingerprint-based criminal history check determination letter is dated 2018.
2. The director stated staff 1 is a rehire and they thought they could use the old fingerprint-based criminal history check determination letter since five years had not passed.
3. The record for staff 2 documents the hire date as 10/16/20. The orientation is documented as conducted on 10/16/20. The fingerprint-based criminal history check determination letter is dated 10/20/20.
4. The record for staff 3 documents the hire date as 11/2/20. The orientation is documented as conducted on 11/2/20. The fingerprint-based criminal history check determination letter is dated 11/4/20.
5. The director verified the dates for staff 2 and 3. The director stated staff 2 and 3 did not start with children until after fingerprint results were received.

Plan of Correction: In the future staff will not be hired until fingerprint-based criminal history check determination letters are received.
