63.2(17) License & Registration Procedures

Technical Assistance:
Discussed with Director: Certificate of Occupancy for the center, potential capacity change, COVID guidance requirements. PPE "pushpack" delivery was also discussed
Request for a Building Inspection form and the most recent COVID guidance document were sent to the Director

Comments:
This Inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 02-17-2021 and concluded on 02-17-2021. The director was contacted by phone to initiate the inspection. There were 56 children present and 13 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 4 of staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on a review of records, two staff records do not contain documentation of a completed nationwide fingerprint search prior to employment.
Evidence:
2 of 4 staff records reviewed did not contain documentation of the fingerprint results prior to the first date of employment.
Staff #1 - Date of Hire (DOH) 10/12/2020- Fingerprint results are dated 10/13/2020.
Staff # 2 - DOH 9/21/2020 - Fingerprint results are dated 9/28/2020.

Plan of Correction: All staff will have fingerprint results on file prior to employment.

Standard #: 63.2(17)-1720.1-B-4
Description: Based on a review of records, one staff record does not contain documentation of a Sex Offender Registry check and a Criminal History check for a state the staff resided in within the past 5 years.
Evidence:
Staff # 2 resided in another state within the past 5 years. The results of a Criminal History Check, and a Sex Offender Registry check were required to be on file by 10/1/2020 for persons newly hired after July 1, 2020. The Sex Offender search results on file are dated 10/28/2020. The Criminal History check is dated 12/11/2020.

Plan of Correction: These results are now on file and current. In the future, all results will be on file prior to employment.
