Technical Assistance:
1. Discussed child enrollment requirements, and why each vendor is to have children enroll at their home. Child enrollment paperwork may not be shared among vendors.

2. Reviewed/trained vendor on new subsidy requirements.

3. Attempted to assist vendor with determining if she had signed the most recent subsidy vendor agreement.

10:40am - 12:15pm Annual unannounced subsidy inspection conducted. There were no children present during the inspection. The home was clean and sufficiently supplied with toys for the children. The vendor is currently certified in First Aid/CPR. Recordkeeping violations found.

Violations:
Standard #: 22VAC40-665-170-A
Description: Based on records reviewed and interview conducted, it was determined that 1 of 2 children recently in attendance did not have a record of enrollment.

Evidence:

1. According to attendance records, the vendor provided care for Child #2 from 7/22/19-8/9/19.

2. There was no record of enrollment for Child #2, and the vendor said she did not have the parent sign a contract or enrollment documents with her. The vendor said she "borrowed" enrollment documents from another vendor, and has since "returned" the paperwork to the other vendor when the child left her care.

Plan of Correction: The vendor said she will not "share" enrollment paperwork with other vendors in the future, and will have parents enroll children with her.

Standard #: 22VAC40-665-410-B
Description: Based on records reviewed and interview conducted, it was determined that an emergency evacuation drill was not completed monthly. Evidence: There was no drill completed in June 2019, and the vendor said she did not practice a drill that month.
