Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on February 15, 2021 and concluded on February 18, 2021. The director was contacted by telephone to initiate the inspection. There were 16 children present and three staff. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed two children's records and four staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children records, the center failed to ensure all required information was obtained and in the record.

Evidence:
1. Child #1's record only contained one out of two required emergency contacts. Date of enrollment for Child #1 was 02/02/2021.
2. Child #2's record only contained out out of two required emergency contacts. Date of enrollment for Child #2 was 01/04/2021.
3. Staff verified that there was only one emergency contact listed in Child #1 and Child #2's record.

Plan of Correction: Administration will contact the parents today and obtain a second emergency contact. In the future when a child is enrolled the forms will be reviewed to ensure all information is provided.

Standard #: 22VAC40-185-70-A
Description: Based on review of staff records, the center failed to obtain reference checks before the first day of employment.

Evidence:
1. Staff #3's file did not contain reference checks. The reference checks had to be obtained from the human resource department. The reference checks were completed on 02/16/2021 and 02/17/2021. Date of employment for Staff #3 was 02/08/2021.

Plan of Correction: Administration will meet with the human resource department and train them on what items are required to have on file before an employee's first day of employment.

Standard #: 22VAC40-185-500-B
Description: Based on interview with the staff, the center failed to ensure that proper cleaning procedures were being completed on the diaper changing table after each use.

Evidence:
1. Staff #4 stated that after diapering a child that the changing table was wiped down with a baby wipe between each child.
2. Staff #4 verified that the changing table was not being cleaned with soap and water, and was not being sanitized with a disinfectant solution and allowed to air dry.

Plan of Correction: Administration will create a step by step poster to be placed in the restroom that outlines the proper procedure for cleaning and sanitizing the dipaer changing area. All staff will receive additional training in regards to diapering and the licensing requirements.

Standard #: 22VAC40-191-60-B
Description: Based on a review of staff records, the center failed to ensure that a sworn disclosure statement was signed and on file before the first day of employment.

Evidence:
1. Staff #3's file did not contain a sworn disclosure statement. The statement had to be obtained from the human resource department. Upon review of the document the sworn disclosure statement was signed on 02/16/2021. Date of employment for Staff #3 was 02/08/2021.
2. Staff verified the date of employment and the date on the signed sworn disclosure statement.

Plan of Correction: Administration will meet with the human resource department and train them on what items are required to have on file before an employee's first day of employment.
