1. Discussed attendance requirements, documenting arrival + departure as it occurs.
2. Reviewed requirement for each child's record to have an emergency contact individual listed, in addition to parent(s). The parent may not be listed as such.
3. Reviewed/trained vendor on new subsidy regulations.
4. Forms provided: new emergency plan, attendance, and instructions for proof of identity.

10:45am-12:30pm Annual unannounced subsidy inspection conducted. Observed 3 children = 8 points with the vendor. Ratios and supervision were in compliance. The children were playing with toys, watching educational children's programs on TV, washing hands, and eating lunch. The vendor demonstrated patience with a newly enrolled infant that was adjusting to the new environment. The daycare areas of the home were found to be clean, safeguarded appropriately, and well supplied with toys and equipment for the children. The vendor is currently certified in First Aid/CPR. Recordkeeping, health, and safety non-compliances found.

Standard #: 22VAC4-665-340-H Description: Based on observations made, it was determined that a crib in-use today does not meet the current federal safety guidelines, as it was not manufactured 5/1/13, or later. Evidence: According to the manufacturer's label on the crib used by Child #6, it was made in February 2012. Plan of Correction: The vendor will get a newer crib.
Standard #: 22VAC40-665-170-B Description: Based on records reviewed, it was determined that 2 of 6 children's records did not include required information.
Evidence:
1. Child #6 - Name, address + phone number for an emergency contact person, other than parents; and written authorization for emergency medical care.
2. Child #7 - Name, address + phone number for an emergency contact person, other than parents.Plan of Correction: The vendor said she will have parents write this information.
Standard #: 22VAC40-665-450-C Description: Based on records reviewed and interview conducted, it was determined that the dog does not have a current rabies vaccine. Evidence: The rabies vaccine on-file expired 10/1/18. The vendor said that the vaccine has not been updated. Plan of Correction: The vendor said she plans to do this on Saturday.
