Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on 2/22/21 and concluded on 2/26/21. The director was contacted by telephone to initiate the inspection. There were 16 children present and 2 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children's records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:

Standard #:	22VAC40-185-550-D
Description:	Based on review of emergency drill records and interview, the center did not ensured that practice evacuation drills are conducted monthly and a minimum of two shelter-in-place drills are conducted per year. Evidence: 1. The emergency drill record provided by the center listed no documented monthly practice evacuation drills for the months of September 2020 through January 2021 and no shelter-in-place drills for the past year. 2. Administrative staff confirmed that no practice evacuation drills were conducted from September 2020 through January 2021 and that no shelter-in-place drills were conducted in the past year.
Plan of Correction:	The center responded with the following: A fire drill was conducted on 2/25/21. Going forward, the center will ensure fire drills are practiced monthly and shelter-in-place drills are conducted twice a year.
Standard #:	63.2(17)-1720.1-B-3
Description:	Based on record review and interview, the center did not ensure that staff who have lived outside of the state of Virginia within the past 5 years obtain a central registry check from that state. Evidence: 1. Staff 1 has a hire date of 1/2/20. Staff 1 indicated on his sworn statement or affirmation that he has resided in the state of Massachusetts within the past five years. There is no documentation that a request for an out of state central registry check was sent to Massachusetts for staff 1. 2. The director confirmed that an out-of-state central registry search was not requested from the state of Massachusetts for staff 1.
Plan of Correction:	The center responded with the following: The out-of-state central registry request has been completed for staff 1 and the results have been placed in the staff record. Going forward, the center will ensure all background checks are completed within the required timeframes.

Description:	Based on record review and interview, the center did not ensure that a copy of the results of a criminal history record check and a sex offender registry check is obtained from any state in which the individual has resided in the preceding five years. Evidence: 1. Staff 1 has a hire date of 1/2/20. Staff 1 indicated on his sworn statement or affirmation that he has resided in the state of Massachusetts within the past five years. The results of a criminal history records check and a sex offender registry check were not available for staff 1 from the state of Massachusetts. 2. Center administration confirmed that the required out-of-state background checks for staff 1 have not been obtained.
Plan of Correction:	The center responded with the following: The out-of-state background sec offender registry check for staff 1 has been completed and the results have been placed in the staff record. The out-of-state criminal history record check request has been filled out and mailed. The results will be placed in staff 1's record once they are received. Going forward, the center will ensure that all required out-of-state background searches are completed within the required timeframes.
