This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on 3/1/2021 and concluded on 3/2/2021. The director was contacted by telephone to initiate the inspection. There were 43 children present and 10 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliances with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-665-540-A Description: Based on record review, the center did not obtain documentation stating that staff are free of communicable tuberculosis (TB) that was completed within the last 30 calendar days of the date of employment.
Evidence: Staff #3 (Date of Hire: 2/12/20) had a TB screening on file dated 10/24/19.Plan of Correction: Have staff get new TB test w/in 30 days
Standard #: 22VAC40-665-580-H Description: Based on record review, the center did not ensure that all staff have completed the Virginia Preservice Update training within one year of their completion of the Virginia Preservice training.
Evidence: Staff #2 (Date of Hire: 9/11/15) completed the Virginia Preservice training on 12/6/16 and completed the Virginia Preservice Update training on 2/11/21. Staff #3 (Date of Hire: 2/12/20) completed the Virginia Preservice training on 10/9/19 and completed the Virginia Preservice Update training on 3/1/21.Plan of Correction: Take training w/in 1 yr of 10 hr. training
