Discussed children's records, medication authorization forms, out-of-state background checks and CPR/First Aid certification.

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 03/02/2021 and concluded on 03/17/2021. The director was contacted by telephone to initiate the inspection. There were 24 children present and 7 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. Standard #: 63.2(17)-1720.1-B-4 Description: Based on record review, it was determined that the center did not ensure that employees that had lived out of the state of VA within the last 5 years have in their files the result of an out-of-state Sex Offender Registry search request, an out-of-state Criminal History Name Check and an out-of-state Central Registry search request.
Evidence:
1) The record for Staff #3 did not contain documentation of an out-of-state Sex Offender Registry search request and an out-of-state Criminal History Name Check. The record for Agent #1 did not contain documentation of a Central Registry search request and an out-of-state Sex Offender Registry search request. Staff #3 and Agent #1 had declared in their sworn statement having lived out of the state of VA within the last 5 years before employment.Plan of Correction: I will follow up with department of IL for an update this week. The Sex Offender search for Agent #1 was completed on 03/17/2021. Will request the Central Registry search ASAP.
