Standard #: 22VAC40-665-520-B-11 Description: Based on documentation review and staff interviews, the vendor did not ensure a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence: The record or child #3 had documentation of a physician diagnosed food allergy (peanuts). There was not a allergy action plan written by a physician in the record.Plan of Correction: Child #3's mom will obtain an allergy action plan when she goes to the dr.
