Standard #: 22VAC40-185-130-A Description: Based on review of three children's records it was determined that not all records contained documentation of immunizations before the first date of attendance. Evidence: Child A (start date: 02/05/2021) and Child B (start date: 02/04/2021) did not have documentation of immunizations on file. Plan of Correction: Administration will request an up to date immunization record from the parents of Child A and Child B.
