Comments:
A self reported inspection was conducted regarding care and protection. This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. The Director was contacted via Facetime, staff and parent phone interviews were conducted and written documentation reviewed. After carefully reviewing all involved parties, some violations were found. Please contact me at: lisa.hudson@dss.virginia.gov with questions.

Violations:
Standard #: 22VAC40-185-340-A
Description: Based on staff statements and center documentation, 1 staff did not ensure the care and protection of one child.
Evidence:
1. The Center's policy for infant feeding requires that before warming a baby bottle, staff are to verify that the name on the bottle matches the child being fed. Before feeding a child , the policy requires staff to verify the name on the bottle matches the child being fed by completing a name to face check. The policy requires that staff verify with another staff in the classroom by showing the other staff the bottle to ensure the name is visible and identify which child is receiving the bottle.

2. Staff #1 admitted to not checking the bottle to verify the name of the child being fed. Staff #1 admitted that they did not verify that the bottle was correct with Staff #2 prior to giving the bottle to Child #1.

3. Staff #1 gave Child #1 a baby bottle that belonged to Child #2. Child #1 consumed the baby bottle with the exception of approximately 1/2 ounce.

Plan of Correction: Staff # 1-Writtten warning
Staff #1- Retraining on Infant Policy and Procedures
Updated Infant classroom labeling system to include color coded labels for each child bottles and food. Each student will receive their own color and all items for that child will be labeled accordingly.

Updated Infant Feeding Policy.

Standard #: 22VAC40-185-560-M
Description: Based on staff statements, 2 staff failed to sit with a child during 1 meal time.
Evidence:
On 02/26/2021, Child #1 was drinking a baby bottle while propped on a boppy pillow on the carpeted area of the classroom. Neither Staff #1 and #2 sat with Child #1 while drinking the baby bottle.

Plan of Correction: Retraining of Staff #1 and Staff #2

Standard #: 22VAC40-185-570-A
Description: Based on staff statements, one infant was not supervised and was not placed in a high chair, infant carrier seat or feeding table while eating.
Evidence:
l. Staff #1 stated that they placed "[Child #1, 10 months] with [a] baby bottle down on the boppy on the carpet".
2. Staff #1 further stated that they went to "change a diaper of another baby" while Staff #2 was feeding Child #2 who was sitting in a feeding chair, leaving Child #1 drinking a baby bottle on the floor unsupervised.

Plan of Correction: Staff #1- Written Warning
Staff #2 Verbal Warning
Retraining of Staff #1 and #2
