Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on 03/08/2021 and concluded on 03/10/2021. The director was contacted by telephone to initiate the inspection. There were 12 children present and 7 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-665-520-B-11
Description: Based on a review of records and interview, the vendor did not obtain a written allergy care plan for two out of three children with a diagnosed food allergy.

Evidence: 1) Child #1 has a diagnosed food allergy, but the record did not contain a written allergy care plan with the required information. 2) Child #2 has a diagnosed food allergy, but the record did not contain a written allergy care plan with the required information. 3) During interview, a member of management confirmed the center does not have a written allergy care plan for Child #1 or for Child #2. A child's record shall include a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction

Plan of Correction: The vendor sent allergy care plan forms home to be completed by each child's physician.
