Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on March 16, 2021 and concluded on March 16, 2021. The administrator was contacted by phone to initiate the virtual inspection. There were thirteen children present with five staff. Two additional children attended before school.
The inspector emailed the administrator a list of items required to complete the inspection. The inspector reviewed two children?s records and three staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined there were non-compliances with applicable standards or law and a violation was documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records, and an interview with administrative staff, the center failed to ensure that each staff record included all documentation required to demonstrate the education and training required for the job position. Evidence: Staff member 1 began employment as a lead teacher on 12/28/2020. There was not a high school diploma in the file to demonstrate the highest level of education. In addition, there was not any documentation of orientation training in topics required before and by the end of the first date of working with children. The administrator indicated that orientation training was conducted and not recorded as it was completed.

Plan of Correction: The administrator is arranging for staff member 1 to submit a copy of the high school diploma that will be kept in the record. The administrator will make sure education diplomas or transcripts are obtained for staff records at the time of hire. Also, an orientation checklist will be used to document orientation of new staff at the time of initial training. A form will be used to record the training staff member 1 received.

Standard #: 22VAC40-191-60-B
Description: Based on a review of staff records, and an interview with administrative staff, the center did not ensure that all staff were not employed until a sworn disclosure or affirmation statement was completed. Evidence: Staff member 2 began employment on January 6, 2021. A sworn disclosure statement was not signed until March 16, 2021 ahead of the staff record review.

Plan of Correction: The administrator will make sure the sworn disclosure statement or affirmation form is completed by all new hires before they begin work and is viewed at the time of hire by the administrator.
