This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on 03/23/2021 and concluded on 03/24/2021. The director was contacted by telephone to initiate the inspection. There were 105 children present and 21 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.
The information gathered during the inspection determined non-compliance with subsidy regulations, and violations were documented on the violation notice issued to the facility.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 22VAC40-665-580-H Description: Based on record review and interview, the vendor did not ensure that each staff complete annually the Virginia Annual Health and Safety Update course, sponsored by the Department of Social Services as required.
Evidence:
1. The record of staff # 5 (DOH: 08/28/2008) contained documentation of the Virginia Annual Health and Safety Update dated 09/27/2020.
2. The vendor acknowledged that the training update was not completed by 03/01/2020 as required.Plan of Correction: Employee has completed the Virginia Annual Health and Safety Update
training.
