Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on 03/24/2021 and concluded on 03/25/2021. The director was contacted by telephone to initiate the inspection. There were 17 children present and 4 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #:	22VAC40-185-130-A
Description:	Based on record review and interview, the center did not obtain documentation that each child had received the immunizations required by the State Board of Health before the first day of attendance. Evidence: 1. Child #2 (DOE:11/06/2017) had an immunization record dated 12/28/2017. 2. Administration confirmed that the immunization record was received after the first day of attendance.
Plan of Correction:	We do have updated immunizations for child #2 and will keep accurate records (punch list) to ensure form is in packet prior to enrollment.
Standard #:	22VAC40-185-140-A
Description:	Based on record review and interview, the center did not ensure that each child had a physical examination by or under the direction of a physician either before the child's attendance or within one month after attendance. Evidence: 1. Child #2 (DOE:11/06/2017) had a physical record dated 12/28/2017. 2. Administration confirmed that the physical was received late.
Plan of Correction:	We do have updated physical record for child #2 and will keep accurate records (punch list) to ensure form is in packet prior to enrollment.
