An unannounced SHSI inspection was conducted today from 8:10 am- 9:30 am. There were 3 children ( 6 points) in care at the time of inspection. The children were observed during parent drop off, watching Tv, having morning snack, completing puzzles, and hand washing routines. A child who was new today was comforted during his new routine. Vendor/ household member, child information was reviewed. As well as emergency preparedness planning, supplies and drills. Standard #: 22VAC40-665-160-B Description: Based on observation and interview, the vendor did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: There was no written attendance for all 3 children in care this morning when inspector arrived. Children that come in the morning need a time in/ time out when dropped off at school and then document again time picked up from school and time parent picks up. Plan of Correction: I will start today.
Standard #: 22VAC40-665-170-B Description: Based on records reviewed, required child information was found to be incomplete. Evidence: Child A who is transported to pre-school does not have a fieldtrip/ permission to transport form signed by parents. Child B who is transported to pre-school does not have a fieldtrip/ permission to transport form signed by parents, sick policy, medication administration policy, emergency contact name address and telephone number and authorization for emergency medical treatment signed by parent. Child C is missing field trip authorization form signed by parent. Plan of Correction: I have parents sign new form. One child is new today. i will have mom finish missing paper work.
Standard #: 22VAC40-665-210-B Description: Based upon records reviewed, required immunization was not obtained by child's first day of attendance. Evidence: Child B( start date 9/11/2020) did not have immunization record on file. Plan of Correction: I will get from mom.
