Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on April 14,2021 and concluded on April 15, 2021. The Center Director was contacted by telephone to initiate the inspection. There were 45 children present and 7 staff. The inspector emailed the Center Director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records, 3 staff records and the background checks for two individuals submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-450-C
Description: Based on observation and staff interview, the licensee did not ensure crib sheets are washed daily.


Evidence: The center director confirmed during virtual inspection that cribs sheets are "washed weekly and checked daily".

Plan of Correction: Center Director will ensure that our staff that works in the infant room informs parents that cribs sheets must be sent home daily. Letter will be sent home to parents.

Standard #: 22VAC40-185-550-D
Description: Based on documentation review and staff interview the licensee did not ensure to implement a minimum of two shelter-in-place practice drills per year for the most likely to occur scenarios.

Evidence: The center director confirmed only one shelter -in- place drill was conducted for the past year.

Plan of Correction: Center Director will ensure that the center does a third shelter in place for the year. Two were done December and June 2020.

Standard #: 22VAC40-185-550-M
Description: Based on documentation review and staff interview the licensee did not ensure to maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence and includes all required information.

Evidence: Five injury reports were reviewed and determined incomplete. Four injury reports did not include the time and how the parent was notified of the injury, two did not include the date the parent was notified of the injury and one did not include any future action to prevent recurrence.

Plan of Correction: Center Director will ensure that each staff is retrained and given sample injury form on how to fill out properly.
