Technical Assistance:
Training information was provided to the Vendor and consultation provided about changes in Regulations.

Comments:
An unannounced inspection was conducted this afternoon. One child was present with the vendor for a total of 2 points. The Vendor does not have children that require medication and is not MAT certified. The Vendor has attended various trainings and is CPR/FirstAid certified. Vendor has paperwork maintained and home has materials for the children. Child was observed playing with blocks and drawing. Please call with questions 703-479-4679

Description: Based on review of child #1's information, it was determined the vendor did not obtain immunization record and completed emergency medical authorization.

Plan of Correction: I will ask for the immunization and have parent complete the authorization form to include my name.

Standard #: 22VAC40-665-400-B-5
Description: Based on review of Emergency Preparedness Plan, it was determined the lockdown procedures did not include methods for accounting for children and continuation of care.

Plan of Correction: Inspector gave the vendor a blank copy and most recent preparedness plan to complete.
