63.2(17) License & Registration Procedures

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on April 27, 2021 and concluded on April 28, 2021. The director was contacted by telephone to initiate the inspection. There were 55 children present and with 14 staff. The inspector reviewed 4 children's rerecords and 4 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child care programs.

Violations:
Standard #: 22VAC40-185-560-F-4
Description: Based on observation and staff interviews, the licensee did not ensure children three years of age or younger were not be offered foods that are considered to be potential choking hazards.
Evidence; During virtual inspection of the two year-old classroom, four two year old children were observed during lunch and whole, uncooked baby carrots were served as a food item for lunch. Center staff confirmed the carrots were not cut up and the children were age 2.

Plan of Correction: We will ensure that children of age or younger will not be offered food that is a potential choking hazards.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review and staff interviews, the licensee did not obtain a fingerprint prior to to first day or employment.
Evidence: Two out of four records reviewed determined, that two staff had obtained a fingerprint check after employment began. Staff #3 first date of employment was 2-1-2018 and finger print check is dated 2-21-2018. Staff #4 first date of employment was 2-12-20218 and finger print check is dated 2-22-20218.

Plan of Correction: We will obtain a fingerprint check prior to the first day of employment of all staff.
