Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on April 28, 2021. The director was contacted by telephone to initiate the inspection. There were 90 children present and 19 total staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed five children?s records and five staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-440-B
Description: Based on observation, cots were not identified for use by a specific child.

Evidence:
1. In the toddler's classroom, none of the cots were labelled for use by a specific child.

Plan of Correction: The cots will be labelled in each classroom.

Standard #: 22VAC40-185-510-A
Description: Based on record review of medication authorization forms, the center did not obtain written authorization from the parent.

Evidence:
1. Child #1 (Start date-12/14/20) has an epinephrine auto-injector(EpiPen) on site. The center has an authorization form that is signed by a physician, but does not have a signature from the parent.

Plan of Correction: The center will have the parents to sign the authorization form today.

Standard #: 63.2(17)-1720.1-B-4
Description: Based on record review and interview, staff record was missing documentation of a sex offender registry check from a state in which the staff member has resided in the preceding five years by the deadline of December 31, 2020.

Evidence:
1. Staff Member #5-(Date of Hire-08/13/2007) lived in the state of Maryland(MD) within the last 5 years. The results of the sex offender registry check were not obtained from or until April 28, 2021.
2. Staff #6 confirmed that a sex offender registry check was not obtained.

Plan of Correction: A Sworn Statement was provided indicating that the staff member presided in the state of Maryland in the preceding 5 years. A Maryland Central Registry Check was obtained on 06/07/2019. The Maryland Sex Offender Registry Check was obtained and verified during the time of the inspection. A copy of the results was dated and placed in the staff record.
