Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on 5/3/2021. The Director was contacted by telephone to initiate the inspection. There were 41 children present and 8 staff. The Inspector emailed the Director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and one violation was documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a review of children's health records, it was determined that the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
Child A entered care on 4/11/2021. There is no shot record on file for this child.

Plan of Correction: I called the parent during the inspection. The child had a physical and updated shots last week. The parent will bring a copy of the health records today at pick up.
