This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 4/16/2021 and concluded on 4/16/2021. The director was contacted by telephone to initiate the inspection. There were 28 children present and 5 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-185-60-A Description: Based on review of three children's files, the facility failed to have all required information in each file. Evidence: Child A did not have documentation of a second name, address, and phone number of a second emergency contact. Plan of Correction: Center director will request documentation of a second emergency contact.
Standard #: 22VAC40-185-340-D Description: Based on review of three staff files, the facility failed to have one person who meets program leader qualifications in each grouping of children regularly present. Evidence: Staff 1 is regularly present in the infant room and does not have documentation of program lead qualifications. Plan of Correction: Not available online. Standard #: 22VAC40-185-350-F Description: A center may choose to assign a child to a different age group if such age group is more appropriate for the child's developmental level and the staff-to-children ratio shall be for the established age group, with a parent's written permission and a written assessment by the program director and program leader. Based on review of the written assessment to assign two toddlers to the two year old classroom, the facility did not document the name of the program leader or the assessment by the program leader. Evidence: Child 1 (age 20 months) and Child 2 (age 21 months) did not have documented assessment by the program leader to move to the two year old classroom. Plan of Correction: Not available online.
