Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 04/28/2021 and concluded on 04/28/2021. The director was contacted by telephone to initiate the inspection. There were 49 children present and 7 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:

Standard #:	22VAC40-185-140-A
Description:	Based on record review, 1 of 3 children's records reviewed did not have documentation that a physical was conducted by a physician within 30 days of the first date of attendance. Evidence: 1. The record for Child #1 (start date 01/05/2021) did not contain documentation of a physical examination.
Plan of Correction:	We will call the parents and have them provide us with the documentation.
Standard #:	22VAC40-185-40-G
Description:	Based on record review, the center did not meet proof of age and identity requirements as stated in the Code of Virginia for one child. Evidence: 1. Documentation in the record for Child #2 (start date 11/16/2020) indicated the original proof of age and identity document was viewed on 2/9/2021. 2. The document was not reviewed within seven business days of the start date for Child #2 as required by the Code of Virginia.
Plan of Correction:	Documentation for the child was viewed and documented in the file. We will be more vehement of getting documentation in within the required deadlines.
Standard #:	22VAC40-185-550-M
Description:	Based on record review, 1 record of injury document did not contain all required information. Evidence: 1. An injury report dated 11/6/2020 did not contain documentation of future action to prevent recurrence of the injury.
Plan of Correction:	We will review the requirement with staff. We will also review our injury form to see if we can add wording to remind staff it is required.
