Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on Tuesday, May 4, 2021 and concluded on Friday, May 7, 2021. The director was contacted by telephone to initiate the inspection. There were 14 children and 6 staff present at the center for in person learning and the program also offers virtual learning. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 4 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Violations:
Standard #: 22VAC40-185-160-C
Description: Based on review of staff record, the center did not ensure all staff resubmit TB test results every 2 years.

Evidence: In Staff#4's record (date of hire: 5/27/99) the last TB test was dated 03/22/19.

Plan of Correction: The Health Coordinator will continue to notify staff 60 days before the PPD expiration.
A second notice will be sent to the staff his/her supervisor, and the Director after 30 days, if not received. If the staff unable to see his/her PCP the agency will refer staff to I&O Medical to expedite the process.
The Health Coordinator will sent a digital copy of the result to HR to file.
The staff letter is attached for review.
