63.2(17) License & Registration Procedures
This inspection was conducted by licensing staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 04/30/2021 and was conducted on 05/10/2021 and 05/14/2021. The provider was contacted by telephone to initiate the inspection. There were 19 children and 6 staff present during the inspection. The inspector reviewed 2 children's records, 2 complete staff records, the background check requirements for 2 newly hired staff, 5 board officer/agent records, and 1 medication during the inspection to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on record review and discussion with the staff, the facility failed to ensure that no person shall be hired for compensated employment prior to the facility obtaining an employment eligibility letter upon the completion of a fingerprint background check as required by the Code of Virginia.
Evidence: The record for Staff #4 has a documented hire date of 09/27/2020; there was not an employment eligibility letter available for Staff #4.
Plan of Correction: If the employee continues their employment a fingerprint background check will be required.
Standard #: 63.2(17)-1721.1-B-4 Description: Based on record review and discussion with the staff, the facility failed to obtain a copy of a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years for all board officer/agents.
Evidence: The record for Board Officer #2 had documentation of having resided in a state other than Virginia within the preceding five years; there was not a sex offender registry check, nor a search of the child abuse and neglect registry or equivalent registry for every state that Board Officer #2 has resided in within the preceding five years available for review.
Plan of Correction: The information will be requested.
