Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A complaint inspection was initiated on April 30, 2021 and concluded on May 14, 2021. A complaint incident was received by the department regarding allegations in the areas of building maintenance, ratios, topical skin products, medication administration, and diapering. The director was contacted by telephone to initiate the investigation. Observations were made, interviews were conducted, and documentation was reviewed.

The preponderance of the evidence gathered during the investigation supports one or more of the allegations and the complaint is determined valid. Violations that were not a part of the original complaint are also listed on the violation notice.

Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to the licensing office within 5 business days. Your plan of correction should contain 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. If you have any questions, please contact the licensing inspector at 804-662-9092.
