Comments:
This inspection was conducted by licensing staff using a combination of an alternative remote protocol and an onsite physical plant inspection, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An initial inspection was conducted on 05/13/2021 and concluded on 05/17/2021. There was 1 staff member present during the inspection. The inspector reviewed and measured the facility and reviewed the required the emergency supplies during the inspection. One applicant record was reviewed during the inspection to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation and discussion with staff, the facility failed to ensure that all areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:
1. The fencing around the outdoor play area had numerous sharp pieces that could snag skin.
2. The tops of the metal posts that held the border around the play area in place were sharp and could snag skin.
3. The paint on the outdoor storage building, that is accessible to children while on the play area, is chipped and peeling.

Plan of Correction: The issues on the fencing will be identified and fixed. New boards/panels will be placed on the outdoor storage building to prevent access to the chipping/peeling paint. Protective barriers will be placed on the metal posts that hold the border in place. Other renovations will be made as identified.

Standard #: 22VAC40-185-540-C
Description: Based on observation and discussion with the staff, the facility failed to maintain the minimum requirements of a first aid kit.

Evidence: The first aid kits did not contain triangular bandages.

Plan of Correction: Triangular bandages have been purchased.

Standard #: 22VAC40-185-550-C
Description: Based on observation and discussion with the staff, the facility failed to ensure that emergency evacuation and shelter-in-place procedures/maps shall be posted in a location conspicuous to staff and children on each floor of each building.

Evidence: There were no posted emergency evacuation and shelter-in-place procedures located in the facility.

Plan of Correction: Procedures have been typed and will be posted.
