Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on April 4, 2021. The program director was contacted by telephone to initiate the virtual, remote inspection. There were 15 children present with seven staff. Children present ranged in age from five months to four years old. The inspector reviewed a list of items required to complete the inspection. The inspector reviewed three children?s and three staff records during the virtual, remote inspection to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-500-B
Description: Based on observation and interview the center failed to ensure diapers are disposed of as required.
Evidence:
1 - There was not a step or hands free diaper trash can in use in the infant room during the inspection.
2 - Staff stated that there was not step or hands free trash can being used in the infant room when changing diapers.

Plan of Correction: We now have a step trashcan in infant room for diapers.

Standard #: 22VAC40-185-510-C
Description: Based on review of center policies the center's medication policies and procedures are incomplete.
Evidence:
1 - There is not a written policy that Include methods to prevent use of outdated medication.
a. An expired Albuterol canister was observed in the center's medication box.
b. The prescription label was dated March 21, 2020 and therefore expired March 21, 2021.
2 - Staff 1 confirmed that the medication prescription had expired.

Plan of Correction: We updated our procedures to include our method of checking every 6 months for dates of medication and paperwork to ensure expiration dates are not missed in the future.

Standard #: 22VAC40-185-550-M
Description: Based on record review the center failed to ensure that staff and parent signatures or two staff signatures are obtained on injury reports.
1 - The injury report provided for review during the inspection did not include staff and parent signatures or two staff signatures.
2 - Staff 1 confirmed that the injury report did not contain the required signatures.

Plan of Correction: We now require injury reports to be signed by staff and parents.
