Standard #: 22VAC40-185-60-A Description: Based on record review and interview, the center did not ensure that each child's record contains all the required information.
Evidence:
1. The records for child 1 (date of enrollment: 4/26/21), child 2 (date of enrollment: 5/10/21) and child 3 (date of enrollment: 3/22/21) do not contain the addresses for the two designated people to call in an emergency if a parent cannot be reached.
2. The center owner confirmed that the records for child 1, child 2 and child 3 are lacking addresses for the emergency contacts.Plan of Correction: The center responded with the following: This was an oversight with the children's registration forms. The children's parents will be contacted to obtain the addresses for the required two emergency contacts.
