Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on May 26, 2021 and concluded on May 26, 2021. The director was contacted by telephone to initiate the inspection. There were nine children present and five staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two children?s records and two staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review of children's records, the facility did not ensure each child's record contained all required information.

Evidence:

The record of child #2, enrolled 03/02/2021, did not contain the addresses for the two emergency contacts listed.

Plan of Correction: Per the director: I will have the parent complete the addresses for the two emergency contacts at pick up.

Standard #: 22VAC40-185-210-A-4-a
Description: Based on review of staff records and interview, the facility did not ensure staff identified as program leaders met program leader qualifications.

Evidence:

1. The record of staff #1 (hired 03/02/2020) identified as a program leader as of 09/14/2020, did not possess documentation of the required twenty-four hours of training within six months before being promoted or beginning work or one month after being promoted or beginning work. Staff #1 had documentation of 22.5 hours in the six months prior to being promoted to program leader.
2. The Director acknowledged staff #1 did not complete the required twenty-four hours of training to met program leader qualifications.
3. The record of staff #2 (hired 01/24/2020) identified as a program leader as of 04/06/2020, did not possess documentation of the required twenty-four hours of training within six months before being promoted or beginning work or one month after being promoted or beginning work.
4. The Director acknowledged staff #2 did not complete the required twenty-four hours of training to met program leader qualifications.

Plan of Correction: Per the director: I will cover the classroom until staff complete twenty-four hours of training within the next 30 days to met program leader qualifications.
