Standard #: 22VAC40-185-550-D Description: Based on a review of records and interview on May 10, 2021, the center did not ensure to implement a minimum of two shelter-in-place practice drills per year for the most likely to occur scenarios.
Evidence: 1. There was documentation of one shelter-in-place drill for 2020.
2. Administration stated she didn't think they had to do a Spring drill due to Covid-19.Plan of Correction: We will make sure we have two shelter-in-place drills each calendar year. We will check the drill book each month to confirm we are executing the in-shelter drill. The executive director and associate director will check to make sure we are meeting the in-shelter standards.
Standard #: 22VAC40-185-550-M Description: Based on a review of records and interview on May 10, 2021, the center did not ensure that written records of children's serious and minor injuries contained all the required information.
Evidence: 1. The incident reports for Child #1, Child #5, and Child #6 did not contain documentation on any future action to prevent recurrence of the injury.
2. Administration acknowledged the incident reports were not filled out completely.Plan of Correction: I will talk with teachers and show them how to fill out the form correctly. We will have the person filling it out and witness double-check to make sure all places are filled out correctly. The executive director or associate director will double check each accident report and make sure it is filled out completely and correctly.
